Nearly every patient already knows roughly what they should be doing. The gap is not information — it is the distance between a plan written in an exam room and a Tuesday evening when you are tired, the fridge is empty and the walk did not happen.
Health coaching closes that gap. It runs alongside your lifestyle medicine care as a structured series of check-ins focused on execution: what actually happened this week, what got in the way, and what the smallest workable change is before we speak again.

How it works
Baseline
We start from the six lifestyle medicine competencies already assessed in your care — nutrition, activity, stress, sleep, connection and substance use — so coaching begins with real data rather than a questionnaire.
Small, specific commitments
Each session ends with one or two changes concrete enough to be answerable yes or no next time. Vague goals fail; specific ones do not.
Regular accountability
Scheduled check-ins keep the plan alive between medical visits, and what is learned there feeds back into your physician's plan.
