Meet the doctor
An Internal Medicine physician providing primary care and advanced primary care services to a diverse population — and a certified Lifestyle Medicine physician who treats the habits behind chronic disease, not only the numbers they produce.
Dr. Gayatri Siram is an Internal Medicine Physician providing primary care and advanced primary care services to a diverse population. She obtained her medical degree from Manipal Academy of Higher Education in India, completed her residency at JFK Medical Center, affiliated with the University of Miami, and went on to a fellowship in Anesthesiology for Liver Transplantation at the University of Miami.
That fellowship is an unusual line on a primary care résumé. Transplant anesthesiology is the practice of holding an entire physiology steady while an organ is exchanged — of reading liver, kidney, heart and clotting together rather than one at a time. It is a habit of mind that transfers directly to the patient in clinic with three chronic diagnoses and five medications, whose problems are not separate problems.
Dr. Siram is also a certified Lifestyle Medicine physician and uses therapeutic lifestyle interventions in the treatment of common chronic diseases. She is board certified in Internal Medicine.

Education & credentials
Medical school, residency, fellowship, and two certifications that shape how the practice runs today.

Medical degree — the foundation of a training path that would carry through residency and fellowship in South Florida.
Residency
Affiliated with the University of Miami
Internal medicine residency: the years spent on the wards learning how adult disease actually presents, rather than how it presents in a textbook.
Fellowship
University of Miami
Subspecialty training in the perioperative care of transplant patients — among the most physiologically complex work in medicine.
Certification
Therapeutic lifestyle intervention
Formal training in using therapeutic lifestyle interventions in the treatment of common chronic diseases.
Board certification
Board certified
The credential that governs the everyday work: adult primary care and advanced primary care for a diverse population.
Philosophy of care
A great deal of adult medicine is decided outside the exam room — at the grocery store, at bedtime, in the hour that was supposed to be a walk. Lifestyle Medicine takes that seriously as clinical work rather than as advice offered on the way out the door. It means the conversation about sleep, food, movement, stress and substance use happens with the same care as the conversation about the medication list, because for the common chronic diseases those are the same conversation.
It does not mean refusing to prescribe. Internal medicine is the discipline of knowing when a drug is the right tool and when it is a way of avoiding a harder discussion. Dr. Siram practices both: therapeutic lifestyle interventions in the treatment of common chronic diseases, alongside the full range of conventional adult primary care.
The other half of the philosophy is continuity. You see the same physician each visit, at either office, and the diagnostics that shape the plan — EKG, spirometry, urine dipstick, blood glucose, lab draws — happen in the room while you are still in it. Patterns get noticed when one person is watching the whole arc.
We are dedicated and committed to keeping patients informed of their health — what a result means, and what it does not.
Traditional insurance visits or extended Direct Primary Care visits. Same physician; the billing model is yours to choose.
In-office diagnostics prevent urgent care trips and deliver high quality care during the visit you already came to.
Commonly treated
Internal medicine covers the whole adult body rather than one region of it. These are the concerns patients most often bring to the office — if yours is not listed, call and ask.
Hospital affiliations
Affiliation matters when a clinic problem becomes a hospital problem. Dr. Siram is affiliated with three South Florida hospitals across Broward and Palm Beach counties.

Additional training
Two areas of the practice sit outside routine internal medicine and required separate certification. Both are performed in office, by the physician.
Approach & access
A short, honest description of how the practice works — so you know what you are walking into.
You see Dr. Siram in Boca Raton and in Pembroke Pines. Your record and your plan travel between the two; you do not restart with a new clinician.
If you would be more comfortable bringing a family member or an interpreter to your visit, you are welcome to — tell us when you book so we can allow the extra time.
The practice is built for adult medicine. Patients aged 17 and above are welcome, whether for a first annual visit or for decades-old chronic disease.
Traditional fee-for-service visits billed through your insurance carrier, or a Direct Primary Care membership with extended visits. The physician is the same either way.
EKG, spirometry, urine dipstick, blood glucose and lab draws are done in office, which keeps decisions in the room and patients out of urgent care.
Some problems do not need a waiting room. Follow-ups, medication reviews and straightforward concerns can be handled by video visit.
