Primary Care 369 is founded to provide exceptional care to all individuals above 17 years. We are dedicated and committed to keeping patients informed of their health. We believe that health is in your hands but we travel with you to make the lifestyle changes and provide care to attain wellness in your lives.

Our patient focused services are directed to treat patients under the traditional model delivering care as a fee-for-service based model using insurance carrier, or provide extended visits through the Direct Primary Care service model. Our use of diagnostics prevents urgent care trips and delivers high quality care.

In-office diagnostics

Answers in the room, not in a second appointment

Our use of diagnostics prevents urgent care trips and delivers high quality care. Everything below is performed at the office during your visit.

Lab draws

EKG

An electrocardiogram recorded in the room, so rhythm and conduction questions are answered during the visit that raised them.

Spirometry

Lung function testing for cough, breathlessness, asthma and COPD — measured here rather than scheduled at a separate facility.

Urine dipstick

Point-of-care urinalysis for urinary symptoms, and as part of routine screening in diabetes and kidney concerns.

Blood glucose

Finger-stick glucose on the spot, so a diabetes conversation can be about today's number rather than last month's.

Lab draws

Blood drawn in office and sent to the laboratory — no second trip, no separate draw station appointment.

Influenza & COVID testing

Rapid respiratory testing during the acute visit, which is the only time the result is useful.

Which tests are appropriate for you is a clinical decision made during the visit. If something needs imaging, a specialist or a hospital, we coordinate that too — see consultations.

Two ways to be seen

Same physician. You choose how it is paid for.

Our patient focused services are directed to treat patients under the traditional model, or to provide extended visits through the Direct Primary Care service model.

Model one

Traditional fee-for-service

Care delivered as a fee-for-service based model using your insurance carrier. You are seen, the visit is coded and billed to your plan, and your copay, deductible and coinsurance apply as your policy defines them.

Model two

Direct Primary Care

For patients who do not carry insurance, or who choose not to use a plan. A periodic membership replaces the claim, and the appointment can run as long as the problem needs.

Our offices

Two locations, one record

Boca Raton is the main office and open six days a week. Pembroke Pines runs on a lighter schedule for patients in Broward County.
