A wellness practice by Dr. Siram
Elevate Wellness 369
Premium wellness delivered with a physician's judgment — not a one-size-fits-all script. Every service starts with a consultation and transparent pricing.
Two ways of paying, one physician
The practice runs two complementary sides. Which one you are in changes how the visit is paid for — not who you see, where you go, or whether the rest of your history is in the room.
Billed to insurance
PrimaryCare369
Your medical home for everyday and ongoing health — annual physicals and preventive screening, chronic disease management, acute visits, telemedicine and lab work. Billed to your plan in the ordinary way.

Elevate Wellness 369
Elective wellness for weight, performance and confidence — medical weight loss, hormone therapy, shockwave, peptides, hair restoration and intimate wellness. Paid out of pocket, with the price known before you start.
You are reading about this side now
Keeping both under one physician is the point. A weight programme that knows your thyroid results, a hormone evaluation that can see your medication list, and a wellness plan that does not contradict what you are already being treated for — that is harder to arrange when the two live in different buildings.
The treatments people drive across South Florida for
Each begins with a consultation, because whether a treatment suits you is a clinical question and not a purchase.
Most requested
Medical Weight Loss
Weight treated as the medical condition it is. The programme starts with laboratory work and a body-composition review, uses GLP-1 medication where it is clinically appropriate, and pairs it with coaching and a maintenance plan so the result holds.
Start with a consultationOften chosen for
- A BMI of 30 or above, or 27 and above with a related condition
- Anyone who has plateaued with diet and exercise alone
- People who want a physician guiding the plan, not a mail-order prescription
Drug-free, no downtime
Shockwave Therapy
Low-intensity acoustic waves stimulate blood flow and tissue repair. It is used two ways here: for stubborn musculoskeletal pain such as plantar fasciitis and tendon injury, and as part of a sexual-wellness plan where circulation is the underlying issue.
Ask whether it suits youOften chosen for
- Chronic joint, tendon and heel pain that has not resolved
- Erectile dysfunction linked to circulation
- Anyone wanting a non-invasive, drug-free option first
Evaluation first
Hormone Therapy
Fatigue, poor sleep, low mood and shifting weight have several possible causes, and hormones are only one of them. The evaluation — history, examination, thyroid and hormone panels — establishes which applies to you before anything is prescribed.
Book an evaluationOften chosen for
- Thyroid function studies and hormone panels, including testosterone
- A plain answer when the cause turns out to be something else
- Prescribed and monitored by an internist, not a single-answer clinic
Every wellness service
All are elective and self-pay. Each page sets out what the treatment involves and what it does not promise.
What we will and will not tell you
Every treatment on this side of the practice is provided only after a medical evaluation. Results vary between people, and nothing here is guaranteed — any clinic that promises you a figure in advance is guessing at it.
Pricing is given up front, before you commit to anything, so the cost is a decision you make with the information rather than a surprise at the end. None of these services is billed to insurance; where the laboratory work behind one of them may be, the office will tell you.
If an evaluation shows that a treatment is not right for you, you will be told that plainly. That is the advantage of having this done by a board-certified internist who also manages your ordinary care.
Book a private consultation
Every service starts with a candid, judgment-free conversation about what you are hoping to address.
