This page explains what primarycare369.us collects when you use it, who else touches that information, and how to have it removed. It covers the website — not the medical record we keep for you as a patient.

The short version
We collect only what our two forms ask for, we email it to the practice so a person can call you back, we do not sell it, and we run no advertising trackers. Please do not describe symptoms, test results or any other medical information in a web form — the forms are not a secure channel for it.
This policy applies to primarycare369.us, the public website operated by Primary Care 369 LLC (“the practice”, “we”, “us”). It describes information collected through this website only.
It does not govern the medical record created when you become a patient, and it does not govern the separate websites and applications we link to — including the patient portal. Those are addressed in sections 5 and 8 below.
The website has two forms — an appointment request and a general contact form. Between them they collect the following, and nothing else:
We do not ask for, and the forms do not accept, dates of birth, social security numbers, insurance policy numbers or payment card details. Basic technical information such as your IP address is processed transiently by our hosting provider to deliver the page and to limit form abuse; it is not used to build a profile of you.
The forms on this website are not a secure channel for protected health information. Messages sent through them travel by ordinary email, which is not encrypted end to end and is not an appropriate place for clinical detail.
Please do not describe symptoms, diagnoses, test results, medications, images or any other medical information in a web form. If you need to discuss something clinical, call the office, or use the patient portal, which is designed for that purpose. If your concern is urgent, call the office. In an emergency, call 911.
If medical information does reach us through a form anyway, we treat it with the same care as any other patient information — but sending it that way is a risk we would rather you did not take.
We do not sell your information. We do not rent, trade or share it with data brokers, advertisers or marketing networks. We do not add you to a marketing list because you filled in an appointment form.
Three outside services touch this website. Each has its own privacy policy, which governs what it does with information it receives.
When you submit a form, its contents are transmitted through SendGrid to the practice inbox, and a copy is emailed to the address you provided. SendGrid processes the message in order to deliver it.
Our location pages embed Google Maps so you can see where the offices are and get directions. Loading an embedded map is a request to Google, and Google may set cookies and receive your IP address as a result. We do not control that, and we receive nothing from it.
New-patient registration and the existing-patient login are hosted by eMedPractice on their own domain. When you follow those links you leave this website, and eMedPractice's own privacy policy and terms apply to everything you do there — including anything clinical.
We may also disclose information where the law requires it — for example, in response to a valid legal process.
This website sets no advertising cookies and runs no third-party advertising or retargeting trackers. There is no advertising pixel on this site, and nothing here follows you to other websites.
Patient portal links deserve a specific mention. No analytics, pixel or conversion event of any kind is attached to the patient portal buttons or links anywhere on this site, and clicking one is not measured or recorded. Those links are also set to send no referrer, so eMedPractice is not told which page of this site you came from.
Embedded Google Maps frames may set their own cookies when they load, as described above. Your browser may also store ordinary technical state needed to display the page. If you would rather block all of it, browser settings and privacy extensions can do so; the site remains usable, though maps may not display.
Form submissions arrive as email in the practice inbox and are kept only as long as they are useful for responding to you and for our internal record of enquiries.
You may ask us to delete a website enquiry at any time. Email help@primarycare369.us with the subject “Website data deletion request”, or call either office. Tell us the name and email address you used so we can find the message. We will confirm when it is done.
One limitation to be honest about: if you have become a patient, information that has entered your medical record cannot simply be deleted on request. Medical records are retained as law and professional obligation require. A request under this policy removes the website enquiry, not the clinical record.
The practice’s clinical records are protected health information and are governed by the Health Insurance Portability and Accountability Act (HIPAA) and by the practice’s Notice of Privacy Practices. That notice — not this page — sets out your rights over your medical information, including how it may be used and disclosed, how to access it, and how to file a complaint.
The Notice of Privacy Practices is provided at registration and is available at either office on request. Ask for it; we would rather you read it.
In short: this website policy covers a web form. HIPAA covers your chart. They are separate documents with separate scopes, and where they overlap, the Notice of Privacy Practices governs your health information.
Primary Care 369 provides care to individuals aged 17 and older. This website is not directed at children, and we do not knowingly collect information from children through it.
If you believe a child has submitted information through one of our forms, contact us at help@primarycare369.us and we will delete it.
If the website changes what it collects or which services it relies on, this page will be updated and the “last updated” date at the top will change with it. Material changes will be described here rather than made quietly.
